- Change of name certificate, if applicable
- Relevant qualification(s): certificate and transcripts
- Working with Children Check (WWCC - Employee) – receipt can be accepted
- NDIS Worker Screening Check (Available at: https://www.service.vic.gov.au/services/national-disability-insurance-scheme)
- Please forward the above to me as you obtain the documents / certificates. If you have booked yourself into a course, please forward me the confirmation of booking receipt.
